PATRICK RADDEN KEEFE is a staff writer at The New Yorker and the author, most recently, of the New York Times bestseller Say Nothing: A True Story of Murder and Memory in Northern Ireland, which received the National Book Critics Circle Award for Nonfiction, was selected as one of the ten best books of 2019 by The New York Times Book Review, The Washington Post, the Chicago Tribune, and The Wall Street Journal, and was named one of the "10 Best Nonfiction Books of the Decade" by Entertainment Weekly. His previous books are The Snakehead and Chatter. His work has been recognized with a Guggenheim Fellowship, the National Magazine Award for Feature Writing, and the Orwell Prize for Political Writing. 這是一本非常精彩詳實的報導文學作品,講述 Sacklar 家族三代靠著不當手法行銷藥物而致富,終至纏訟的事件。全書的重點雖在於揭發 Sacklar 家族第二代行銷疼始康定的始末,本無須額外提及家族第一代的歷史,但正因為作者花了不少篇幅描述第一代事業發展「上樑不正」的作為,讓讀者更能理解家族第二代何以能為自身圖利,而漠視(甚至戕害)一般民眾健康,而絲毫不以為恥。

Sacklar 家族第一代是以 Arthur 為首的三兄弟,他們雖然出身貧困,但靠著 Arthur 的生意頭腦與勤奮不懈,於二戰後創立廣告業,替當時的大藥廠刊登廣告行銷新藥。創業沒什麼不對,但他們採取的廣告手法是在醫學期刊上刊登不實的藥效敘述,並捏造醫生簽名背書以推銷新藥,誘導醫師開立新藥處方簽,幫助藥廠大發利市,而三兄弟也因此致富。

家族第二代不只有樣學樣,更青出於藍,把行銷策略發揮到極致。1996 年,Purdue Pharma (下稱「普度製藥」)推出止痛藥疼始康定,一種以緩釋塗層包覆羥考酮(鴉片類藥物)的藥錠。普度製藥向 FDA 申請疼始康定的上市許可時,宣稱疼始康定的緩釋塗層可減緩藥物釋出,降低患者對鴉片類藥物成癮機率,但並未提供符合上述主張的實驗數據,而是以賄賂審查官員的方式取得上市許可。

許可取得後,普度製藥派業務向醫師發送不實的藥效說明,搭配送禮行賄,誘使醫師大量開立疼始康定給一般術後疼痛或其他慢性疼痛的求診者,並積極追蹤全美各地醫師開立疼始康定的情況,對「開藥不力」的醫生加強行銷,利誘醫師開立數量更多、劑量更高的疼始康定處方。而那些因疼痛就醫的患者,則在不知不覺中對止痛藥成癮,傾家蕩產只為了再拿一些疼始康定,當再也買不起價格高昂的疼始康定,就轉尋非法但較便宜的海洛因及吩呔尼解癮,甚至因藥物過量而喪命。

對照無辜病患的藥物成癮,是普渡製藥獲益逐年飆升,疼始康定甚至一度熱賣到供不應求,而這波止痛藥狂銷的真正獲利者,就是普度製藥背後神秘的經營者— Sacklar 家族。Sacklar 家族多年來從不揭露他們與普度製藥的關聯,在外以神秘而富有的慈善家形象自居,對博物館、名校提供巨額捐款,換取在博物館及校內建築物上留名。直到美國各地出現大量因鴉片類藥物過量死亡的案例,政府著手調查,才終於讓普度製藥背後的 Sacklar 家族曝光。

讀本書的過程心情很複雜,一方面因為作者文筆極佳,可說是近幾年來看過最引人入勝的報導文學作品,讀來十分過癮;但在閱讀的過程中,又深刻地意識到作者所描述的每一樁案例都是真實事件:那些因職業工傷,或是搬重物而背部拉傷的普羅大眾,求助醫生只是為了拿到止痛藥,期望吃了之後晚上可以好好睡上一覺,沒想到藥吞了下去,卻陷入成癮噩夢中無法抽身;或是藥廠間接「建議」醫生開藥給來路不明的成癮者,提供他們疼始康定,讓他們繼續在毒癮裡越陷越深,看到這些案例,很難不感到心痛。

這是本值得一讀的好書,推薦大家。
